I have a cheerleading squad and I need to create a photo/name release form. I want it to be something that the parents sign to release their child’s information to the newspapers, newsletters, parades etc. I just want to be able to release their information.

you really just need to type of a slip of paper that says:
“I give my permission for my son/daughter to participate in any public events. I also give permission for my son/daughter to be photographed and for his/her name to be used in any public media such as newspaper, parades, etc…”
